Change of Status - Application Process

 

  1. **Login to your profile first
  2. Complete the online Application for a Restricted License
  3. Proof of $5,000,000 Professional Liability Insurance
  4. Letter of Good Standing from their current or most recent licensing body
  5. Supervision Agreement
  6. Provide a completed current Criminal Record Check
  7. The required fees (subject to change without notice): 
    $500.00 Initial registration and Membership fees (after October 1: $250.00) 
    $75.00 non-refundable initial registration fee, $75 monitoring fee
  8. Total = $650.00 CD ($400.00 after October 1)

 

Once your online applicatiion has been submitted and upon SCPT's approval, a licence will be issued.
