Status Change - Application Process
- **Login to your profile first
- Complete the online change of registration status.
- Your supervision agreement will be terminated, you will need to submit an updated Supervision Agreement and Liability insurance form upon your return
- Upon renewal, you will renew your license as a Non Practicing member and pay the Non Practicing fee of $125.
- If, on renewal, you need to change to Restricted, please complete a change of status for Non Practicing to Restricted and follow those guidelines.
